Youthhealthtalk enables young people, their family and friends, and professionals such as doctors and teachers to understand young people's experiences of health, illness and life in general. The website feature real-life accounts of issues such as effect on work and education, social life and relationships, consulting health professionals and treatment.

New Patient Registration

registrationWe are happy to accept new registrations from a wide area surrounding Sible Hedingham. Please ask the receptionist for a registration pack. This contains a short form to be completed to register with the practice. You are registered with the practice rather than with an individual GP. Your medical card will be issued in the name of one of the doctors.

As surgery policy we ask you to complete the Healthcare Questionnaire as soon as possible. The information will provide us immediately with details of your medical history which would otherwise remain unknown to us until your records come through from your previous doctor.

You will be invited to have a New Patient Health Check usually with one of the nurses, but if you are on repeat medication this will be booked with a doctor.


 You may print off a registration form, fill it out and bring it in with you on your first visit to the practice.
